About Trinity Hospice Foundation
|
The Trinity Hospice
Foundation is non-profit foundation dedicated to the education and
support of hospice care and benefits. The Foundation was founded and
is funded through donations of friends and family of loved ones who
have experienced hospice care and by Trinity Hospice LLC. We serve all
through funding of indigent hospice care, projects for hospice
education, and special programs with relation to hospice.
Major areas which the foundation supports
are:

Funding for
indigent hospice care and providing increased access to hospice for
those less able to afford hospice care, without regard to the hospice
that a patient has chosen.

Specific needs that can be addressed
include: groceries, utility assistance, rent assistance, home repairs,
car repairs, transportation and other needs generally fall outside the scope of other
traditional funding sources like Medicare, Medicaid, or private
insurance. Application for awards must be received in writing
and clearly document the need and other forms of assistance requested.

Community awareness programs can include
written materials, videos, speakers, classes, or seminars that allow
the public to have a better understanding of hospice services and
issues relating to the end of life.

Support of special community-based
projects that provide services to hospice patients or professionals in
the field of hospice services.

Donation Information

The Foundation is a nonprofit corporation
and donations are tax deductible as allowed by law. We are
supported entirely by the generosity of individuals like you who
believe that hospice services are important and can greatly alter our
personal experiences in dealing with the end of the life of a loved
one. Donations may be made in
memory of loved ones or in honor of someone who made a difference in
your life.
Types of gifts:
| Heritage Society - a planned gift such
as a bequest in a will, annuities, life insurance policies, or
other assets that are given to build a perpetual resource for
hospice assistance. Donors are listed in perpetuity.
Memorial donors are listed in our annual report each year in the
following categories:
Gifts of Love - $5,000 and more
Gifts of Remembrance - $1,000 - $4,999Gifts of
Promise - $500 - $999
Gifts of Faith - $100 - $499
Gifts of Friends - $25 - $99 |

Most gifts may be by cash, check, or credit card. Stocks,
securities, appreciated property, cars, real estate, or other assets
are accepted upon approval of the board of directors. All gifts

The Board of
Directors
Dr. Holbrook Lawson
Founder and Chair of the Board
|
Holbrook Lawson has a PhD in Curriculum & Instruction with
a minor in Gerontology. She firmly believes in giving the best
physical, emotional and spiritual care to the elderly. Her
primary work experience has been in the field of hospice where
she is dedicated to making a difference in the lives of
terminally ill patients. To increase community awareness of
elderly issues, she co-hosted 32 half hour radio shows, called
Senior Time. These shows aired in three different states. Dr.
Lawson is currently involved in Trinity Hospice where her
community service and work experience has blended to better
serve older adults. |
